Phyllis Tackett

Phyllis Tackett, 74, of Flatwoods, Kentucky passed away June 3, 2016 at Cabell Huntington Hospital in Huntington, West Virginia.

Mrs. Tackett was born May 28, 1942 in Ironton, Ohio a daughter of the late Jesse and Hattie Kelly McClain.

In addition to her parents she was preceded in death by several brothers and sisters.

She is survived by

Dwayne Tackett of Augusta, Georgia

One daughter

Mary Ealey of Ashland, Kentucky

Three grandchildren

Two great grandchildren
